Based on @Sylvia list above, does this mean Amber and Courtney have great shots at a GP invite

No better than anyone else near the top of the list because host nations don't have to go down the sub list in order. In fact, being a medal "threat" can make one less likely to be chosen.

7.6 Replacements
It is mandatory that an Organizing ISU Member replaces any Pair who withdraws from the event with an alternate up to fourteen (14) days prior to the Team Leaders Meeting for that event.

For a Ladies or Men Skater, up to fourteen (14) days prior to the Team Leaders Meeting for that event, it is voluntary that an Organizing ISU Member replace any Skater who withdraws from the event who would be the 11th or 12th entry, but must replace with an alternate any Skater who withdraws if fewer than 10 entries remain.

For an Ice Dance couple, up to fourteen (14) days prior to the draw for that event, it is voluntary that a host country replace any Ice Dance couple who withdraws from the event who would be the 9th or 10th entry, but must replace any Dance couple who withdraws with an alternate couple if fewer than 8 entries remain.

At the fourteen day mark, consideration will be given by the ISU Grand Prix Coordination Group to leaving a position vacant because of visa requirements or unduly high travel costs that would be incurred by the Organizing ISU Member.

Skater/Couples, who are part of the official Alternate List must be ready to accept an invitation for participation in an ISU Grand Prix of Figure Skating event, even on short notice, except if the reasons for refusal are obvious (injury, illness, conflict with other competitions or with national activities, e.g. seminars, tests, etc.). The benchmark of such short notice is seven (7) days.

The alternate list needs to be worked through in ranking order for the top 75 ranked Skaters/Couples according to their Season’s Best. All Skaters/Couples of the alternate list will be grouped in top ranked groups of ten (10) Skaters, out of which the respective Organizing ISU Member may pick one Skater/Couple for invitation.

Skaters/Couples, who place first in any of the ISU Challenger Series events for 2016/17 and can prove to have skated the minimum total score or the minimum Technical score as mentioned in paragraph 2.3, as well as Skaters/Couples, who can prove to have skated the minimum Technical score will be added to the alternate list, bottom ranked according to their scores.

It is not as if there is such a big choice for the lady. Feditchkina was at JGP, Konstantinova is due to the next JGP event, so surely the GP would be given to Sakhanovich or maybe Mikhailova. Thre is no one else.

I think Amber Glenn is high up on the substitute list. Judging by her performance just now at Lombardia, she may get selected

They could still get the host spot at SA couldn't they. Not saying they will, but would that be possible?
That's an interesting question. If USFS hasn't submitted the names yet, sure, because the rules are:

k) Non-Seeded Skaters/Couples will not be invited to another event in replacement if they choose not to
skate in an assigned event.
l) Non-Seeded Skaters/Couples will not be invited to another event in replacement if, by medical reason,
they withdraw from an assigned event

and, presumably, this would be their second assignment.

But my understanding is that it's USFS policy that TBD spots are filled by skaters/teams who are invited to Champs Camp and based on summer monitoring, although I'm sure they can grant whatever exceptions they want. As of O'Shea's 7/31 blog post, Kayne was still recovering from surgery, and they had just finished choreographing their SP and FS.

Host spots are under the "Invited Skaters/Couples" section, so following the letter of the Announcement -- always a mistake -- they couldn't replace a different Pair who was assigned the TBD. But I don't think the ISU interferes much in host spot assignments.

And Japan's host picks were added: Miu Suzaki/Ryuichi Kihara (pairs) and Misato Komatsubara/Tim Koleto (dance).

ETA:

France added Lola Esbrat/Andrei Novoselov as their 2nd pair team and Lorenza Alessandrini/Pierre Souquet as their 3rd dance team.

That leaves only the CoR ladies TBD (also Ivett Toth withdrew and they haven't named a replacement yet, if they're going to) and the CoC men and pairs TBD spots.

Both Cain/Leduc and Stellato/Bartholomay (who got the SA TBD) were invited to Champs Camp. I wonder if Kayne/O'Shea's impending withdrawal was already communicated to the Chinese Fed, and if it was already a done deal, if not formal, that C/L would replace K/O.
