There's a caption error: The death drop photo is from 2005 (Spartacus program), not 1995, Nationals! The location is correct though.

"Michelle Kwan competing in the women’s short program during the 1995 U.S. Figure Skating Championships in Portland, Ore. Kwan finished her career with nine U.S. titles."

I hope it was just a typo, with the fingers on 1 instead of 2 and 9 instead of 0. Though that doesn't explain why the 5 was correct.
